What to Bring
For new patients and consultations please submit the following prior to your scheduled visit.
- Copies of medical records of any cardiac related tests or procedures such as echocardiography, stress testing, catheterization films, CT/MRI scans and/or EKG results.
- You may also sign this
Medical Records Release Form and our office will request the records directly.
On the day of your visit, for both new and established patients, please bring:
- A list of all the medications that you are taking (include names and dosages)
- Your preferred pharmacy name, address, phone and fax
- Your insurance cards
- Contact informtion of your referring physician and primary care physician
- A list of any questions or your concerns that you may have
What to Expect
Please arrive on time for your scheduled appointment and check-in at the front desk. Our front-desk staff may need you to verify certain demographic information, insurance data and will require your signature on certain privacy notification and release forms. At this time, we also will ask you to pay any co-payment if applicable.
Once in the exam room, our clinical technicians will take your blood pressure, weight, and review with you your medication list. You may be reffered for an EKG. Your doctor will evaluate your medical history, any prior test results, and conduct a physical exam. Your doctor will discuss the results with you and recommend a treatment plan and possible additional testing which can be scheduled upon your check out. We encourage you to ask questions – you are often the key to your cardiovascular health.
We also have an excellent staff of nurse practitioners, registered nurses, clinical technicians and patient coordinators who are here to make your visit as comfortable as possible and to help you with any questions or concerns that you may have.

Long Island City

Our practice is easily accessible from Brooklyn, Queens and throughout Long Island.
If you are coming by subway, you may take the E, M, or R line to Queen’s Plaza or the 7, N, or W line to Queensboro Plaza.
If you are coming by bus, you may take the Q60 or Q32 to Queens Plaza South/28th St or Q101 to Jackson Ave/Orchard St.
Parking
If you are coming by car, discounted parking is now available at One Parking Garage for patients at their two convenient locations, 28-30 Jackson Ave, Queens, NY 11101 and 42-09 28th St, Queens, NY 11101. Enjoy an Early-Bird Special rate of $17 when you park from 5 a.m.-11 a.m. After 11 a.m. it is a flat fee of $20 for patients. Patients must present proof of their same-day appointment. This may be done by providing a screenshot of the appointment via MyChart or a printed copy of their appointment details.
Lower Manhattan

Directions
If you are coming by subway, you may take the 2/3, A/C or J trains to Fulton St, E or 1 trains to World Trade Center, 6 train to Brooklyn Bridge-City Hall, or R or W trains to City Hall.

Directions
By Subway
Take the Q train to 72nd Street and walk two blocks east to York Avenue.
By Bus
Cross Town Bus: Take the M31 to the East 69th Street stop, directly in front of NewYork-Presbyterian/Weill Cornell. The M31 operates north and south on York Avenue, and across town on 57th Street. Cross-town buses M30, M66, and M72 allow you to transfer to the M31 at York Avenue.
Uptown and Downtown Bus: Take the M15 bus which travels uptown along 1st Avenue and downtown on 2nd Avenue. Exit at 68th Street. Either transfer to the M66 line eastbound to York Avenue and 68th Street, or walk 1 or 2 blocks east to the hospital.
From Port Authority Terminal: Take the #10 bus on 8th Avenue going uptown. Get off at 65th Street. Transfer to the #66 bus going east. Get off at 68th Street and York Avenue.
By Car
Approaching from South of East 68th Street: Take the FDR Drive northbound to the 61st Street exit. Make right onto York Avenue and go north to 68th Street.
Approaching from North of East 68th Street: Take the FDR Drive southbound to the 71st Street exit. Make left onto York Avenue and go south to 68th Street.
From New Jersey: Take the George Washington Bridge (lower level) to the Harlem River Drive. Then take the FDR Drive south and exit at 71st Street. Turn right on 71st Street. Go one block and turn left onto York Avenue.
Parking
We strongly recommend parking at our licensed and insured hospital parking garages listed below. Most major credit cards are accepted.
The Greenberg Pavilion Garage

Valet parking is located in the circular driveway at the main Hospital entrance. Payment may be made using the kiosk located near the Cashier’s Office. Monday through Friday, 8 am to 6 pm, patients and visitors can also make payment to the Hospital Cashier. After 6 pm and over the weekend, patients and visitors can use the kiosk or pay valet parking attendants directly.
