Information for Referring Physicians

To refer a patient to see one of our physicians, healthcare providers may contact our office by calling the numbers listed below, weekdays between 9 am and 5 pm.

For non-urgent matters during evenings or weekends, kindly leave a message and a representative will return your call.

If you are a Cornell physician, you may also refer to our services by using an EPIC order and selecting “Consult to Cardiology.” 

Our representative will answer your questions about:

• Physician credentials
• Subspecialty expertise
• Insurance and managed care plans 

At the time of your call, you will be asked to provide the following information:

• Patient's name
• Address
• Date of birth
• Phone number
• Email address
• Insurance information
